Retailer Application
The cost of a Retail liquor license is $500. You must have your
local liquor license, Sales Tax number /Illinois Business Tax
(IBT) number, and Federal Employer Identification Number (FEIN)
before you can receive your state retail liquor license. A certificate
of insurance is required for licensees with on-premise consumption
or a combination of on/off-premise consumption. You must also
provide a photocopy of your local license. NOTE: A receipt
from the purchase of your local license is not acceptable.
Special
Use Permit
A Special Use Permit is required for current retail liquor license
holders to hold any special off-premise event(s). The cost of
the license is $50 for a one day event, $100 if the event's duration
is two days or more up to a maximum of 15 consecutive calendar
days. There is a $25 late fee if the license is applied for less
than 14 days before the event. Local approval and Dram Shop insurance
are required.
Special
Event Retailer
The cost of the license is $25. There is a $25 late fee if the
license is applied for less than 14 days before the event. You
must prove Not-for-Profit status and provide a physical address
for the location for the event. You may not use a P.O. Box or
a Rural Route for the organization's address (a P.O. Box can be
used for the mailing address only). Local approval and Dram Shop
insurance are required.
Specialty
Retailer Application
A Specialty Retailer license includes the following license classes
and fees: Brew Pub$1,050; Caterer Retailer$200; Wine
Maker Retailer$100; and Wine Maker Retailer (Second Location)$350.
The application includes instructions and lists all supporting
documents necessary to obtain the license.
Specialty
Liquor License Application
A Specialty liquor license applies to the following license classifications
and includes the corresponding fees: Auction$50 per auction;
Airplane$60 per plane; Boat$180; Railroad$60
per dining, lounge, buffet, and/or club car. The application includes
instructions and lists all supporting documents necessary to obtain
the license.
Distributor
Application
The cost of the license is $270 for a Distributor only; $295 for
an Importing Distributor's License; $295 for a Foreign Importer's
License; and $320 for a Foreign Importer/Importing Distributor's
Lisense. If you import product from both out-of-state and out-of-country,
you will have to pay the initial $270 plus $50. REQUIRED:
You must provide the Liquor Control Commission with
a copy of your Federal
Basic Permit (click on link to download application),
a warehouse address, and one of the following Illinois Dept. of
Revenue Bond Forms: RL-1 (Liquor Tax Statement of Liability);
REG-4-D (Financial Institution Irrevocable Letter of Credit Bond);
or REG-4-A (Financial Responsibility Bond).
Winery Shipper's License Application
A winery must have this license prior to directly shipping a maximum of 12 cases of wine per adult Illinois resident per year. The cost of the license is $150 for wineries manufacturing under 250,000 gallons annually; $500 if manufacturing 500,000 gallons or less a year; and $1,000 if manufacturing over 500,000 gallons a year. REQUIRED: You must provide the Liquor Control Commission with
a copy of all Federal
Label/Bottle Approvals (click on link to download
application) and one of the following Illinois Dept. of
Revenue Bond Forms: RL-1 (Liquor Tax Statement of Liability);
REG-4-D (Financial Institution Irrevocable Letter of Credit Bond);
or REG-4-A (Financial Responsibility Bond).
Self-Distribution Exception
Required for wineries to self-distribute their product to Illinois retailers. Those receiving this exception must produce 25,000 gallons of wine or less per year and sell 5,000 gallons of wine or less per year to retail licensees.
Non-Resident
Dealer Application
A Non-Resident Dealer's License shall allow the shipment and warehousing
of alcoholic liquor in this State from any point outside of this
State, and to sell such alcoholic liquor to Illinois-licensed
distributors and to no one else in this State; provided the non-resident
dealer registers with the Illinois Liquor Control Commission each
and every brand of alcoholic liquor it proposes to sell to Illinois
licensees during the license period. The cost of the license is
$90 for less than 500,000 gallons combined plant total, or agent
thereof AND/OR Primary U.S. Importer exporting less than 500,000
gallons into Illinois yearly, or agent thereof. The cost of the
license is $270 for a manufacturer of 500,000 gallons or more
per year combined plant total, or agent thereof AND/OR Primary
U.S. Importer exporting 500,000 gallons or more into Illinois
yearly, or agent thereof. REQUIRED: In
addition to the above application and its supporting files, you
must provide the ILCC with copies of your Federal
Basic Permit (click on
link to download application) and all Federal
Label/Bottle Approvals (click on link to download
application).
Registration
Statement
A Registration Statement is required for granting the
right to sell or distribute alcoholic liquors at wholesale within
the State of Illinois.
Manufacturer
Application
A Manufacturer's license consists of the following license classes
and fee amounts:
- Class 1: Distiller$3,600;
- Class 2: Rectifier$3,600;
- Class 3: Brewer$900;
- Class 4: 1st Class Wine Manufacturer$600;
- Class 5: 2nd Class
Wine Manufacturer$1,200;
- Class 6: 1st Class Wine Maker$600;
- Class 7: 2nd Class Wine Maker$1,200;
- Class 8: Limited Wine Manufacturer$120;
- Class 9: Craft Distiller$1800.
REQUIRED: In addition to the
above application and its supporting files, you must provide the
Illinois Liquor Control Commission with copies of your Federal
Basic Permit (click on link to download application)
and all Federal
Label/Bottle Approvals (click on link to download
application).
Manufacturer's
Registered Agent Form
An application to represent a manufacturer as its registered agent.
Broker
Application
The cost of a Broker's License is $600. A broker's license is
required for any person who solicits orders for, offers to sell
or offers to supply alcoholic liquor to retailers in the State
of Illinois, or who offers to retailers to ship or cause to be
shipped or to make contact with distillers, rectifiers, brewers
or manufacturers or any other party within or without the State
of Illinois in order that alcoholic liquors be shipped to a distributor,
importing distributor or foreign importer, whether such solicitation
or offer is consummated within or without the State of Illinois.
Non-Beverage
User Application
A Non-Beverage User's license allows for the purchase of alcoholic
liquor from a licensed manufacturer or importing distributorwithout
the imposition of any tax upon the business of such licensed manufacturer
or importing distributor. The alcoholic liquor can be used solely
for the non-beverage purposes set forth in Section 8-1 of the
Liquor Control Act. A Non-Beverage User's license consists of
the following license classifications: Class 1not to exceed
500 gallons; Class 2not to exceed 1,000 gallons; Class 3not
to exceed 5,000 gallons; Class 4not to exceed 10,000 gallons;
and Class 5not to exceed 50,000 gallons.
Tasting
Representative
A Tasting Representative License allows for the supervised presentation
of alcoholic beverages off-premises for the purpose of disseminating
product information and education.
BASSET
Application
The Beverage Alcohol Sellers and Servers Education and Training
(BASSET) license is $250 annually. You must submit a copy of your
training curriculum, the pre- and post-tests, a copy of the certificate
of course completion, along with your Sales Tax/ Illinois Business
Tax (IBT) number, and Federal Employer Identification Number (FEIN)
in order to receive your BASSET license.
Change of Hours of Operation
Use this form to change the hours of when your business is actually open and available for inspection by the Commission’s Field Agents.

Day Waiver Application
A fee of $10 is charged per location. In order for all staff and/or
personnel to know where the document is kept at the premises,
the original form must be displayed near the State Liquor License.
Duplicate
License Application FormLoss, theft or destruction form
This is different from the "Change of Business form" in that no
information changes are made to your license. You are only receiving
a replacement license due to loss, theft or destruction of your
current State Liquor License. The fee is $12 for a license class
with a standard license fee under $200 or $24 for a license in
a class with a standard license fee which exceeds $200.

of Business Name Application
If you are the owner and you are changing any of the following: Business Name, Corporate Name, Address, Illinois Business Tax (IBT) Number, Business Type, and/or Special Event Information of your business (DBA) you must fill out this form.
Delinquency
Affidavit
This form is required to renew a liquor license that has expired.
Change
of Corporate Officers
You can update your ownership information by completing this form
and submitting the requested supporting documents.
One-Time
Transfer of Alcoholic Liquor Form
A request by the licensee to allow for a one-time transfer of
alcoholic liquor between licensed premises.
Withdraw
of Registration
This form is needed to withdraw the right to sell or distribute
alcoholic liquors at wholesale within the State of Illinois.
Test
Marketing Notice
This form is needed to conduct marketing research of alcoholic
beverages within the State of Illinois.
