INTERNET PRIVACY POLICY
Healthcare and Family Services, Office of Inspector
General (hereinafter referred to as "OIG"), is an
independent governmental agency, whose mission includes enabling
individuals and organizations to establish trusting relationships
based on respect for personal identity and information by
promoting the use of fair information practices. This privacy
statement covers the site www.state.il.us/agency/oig. The
OIG Website has a desire to demonstrate its commitment to
its users' privacy, thus it has agreed to disclose its information
practices, including:
1. What personally identifiable information the OIG collects;
2. How the OIG uses the information;
3. With whom the OIG may share user information;
4. What choices are available to users regarding collection,
use and distribution of the information;
5. What types of security procedures are in place to protect
from the loss, misuse or alteration of information under the
OIG's control;

Information Collection and Use

Access to the OIG's Website is provided subject to the following
terms and conditions. Please read these terms carefully as
use of these sites constitutes acceptance of all of the following
terms and conditions:
Information Collection
The OIG is the sole owner of the information collected on
www.state.il.us/agency/oig. The OIG may collect information
from its users at several different points on its Website.
Information Use
You do not have to give us personal information to visit our
Website. If you choose to provide us with additional information,
including personally identifiable information (name, E-mail
address, physical address and other unique identifiers) about
yourself through an E-mail message, form, survey, etc., we
will only maintain the information as long as needed to respond
to your question or to fulfill the stated purposes of the
communication.
We use personally identifying information obtained from www.state.il.us/agency/oig
in various ways to further the overall mission of the OIG,
including detecting, preventing and eliminating fraud, waste,
abuse, misconduct and mismanagement of programs administered
by Healthcare and Family Services and the Illinois
Department of Human Services. We collect this information
under the authority of Healthcare and Family Services Code and other
laws we enforce or administer. We may enter the information
you send into our database to make it available to attorneys
and investigations involved in program enforcement. We also
may share it with a wide variety of other governmental agencies
charged with the duty of enforcing laws and regulations affecting
Healthcare and Family Services and the Illinois Department
of Human Services. If you contact us because you are reporting
another type of violation, we may share this information with
other law enforcement agencies or governmental entities. In
addition, when you submit a complaint, you may be contacted
by the OIG or any of the agencies to which your complaint
has been referred.
In other limited circumstances, information collected and
received through the OIG Website may become public record
and, therefore, subject to disclosure under the Illinois Freedom
of Information Act (5 ILCS 140/1 et seq.).
We want to be very clear: We will not obtain personally identifying
information about you when you visit our Website, unless you
choose to provide such information.
E-Mail
While the OIG will make every attempt to protect the personal
information that you share with us, electronic mail is not
secure against interception. If your communication is very
sensitive, you may want to send it by postal mail instead.
Data Collection
We may collect and store: the name of the domain and host
from which you access the Internet (for example, aol.com or
uic.edu); the Internet protocol (IP) address of the computer
you are using; the browser software you use and your operating
system; the date and time you access our site; and the Internet
address of the Website from which you linked directly to our
site.
We use this information to measure the number of visitors
to the different sections of our site, and to help us make
our site more useful.
Cookies
We do NOT use cookies on this site. A cookie is a piece of
data stored on the user's computer tied to information about
the user.
Log Files
Like most standard Website servers, we may create and use
log files. These include internet protocol (IP) addresses,
browser type, internet service provider (ISP), referring/exit
pages, platform type, date/time stamp, and number of clicks
to analyze trends, administer the site, track user's movement,
and gather broad demographic information for aggregate use.
IP addresses, etc. are not linked to personally identifiable
information. The OIG's Website logs are not personally identifiable.
Legal Disclaimer
Though we make every effort to preserve user privacy, we may
need to disclose personal information when required by law
wherein we have a good-faith belief that such action is necessary
to comply with a current judicial proceeding, a court order
or legal process served on our Website. We do not share personal
information with third-parties.

This Website contains links to other sites. Please be aware
that we, the OIG, are not responsible for the privacy practices
of other sites. We encourage our users to be aware as to when
they leave our Website and to read the privacy statements
of each and every Website that collects personally identifiable
information. This privacy statement applies solely to information
collected by this Website.
Surveys
From time-to-time our site may request information from users
via surveys. Participation in these surveys is completely
voluntary and the user, therefore, has a choice whether or
not to disclose this information. The requested information
typically includes contact information (such as name and shipping
address), and demographic information (such as zip code).
Survey information will be used for purposes of monitoring
or improving the use and satisfaction of this site. Users'
personally identifiable information that is obtained via a
survey is not shared with third parties unless we give prior
notice and choice. Though we may use an intermediary to conduct
these surveys, they may not use users' personally identifiable
information for any secondary purposes.
Security
The OIG takes every precaution to protect our users' information.
When users submit sensitive information via the OIG Website,
their information is protected both online and offline. The
OIG, as developer and manager of this Website, has taken several
steps to safeguard the integrity of its communications and
computing infrastructure, including but not limited to, authentication,
monitoring, auditing, and encryption.
Security measures have been integrated into the design, implementation
and day-to-day practices of the entire OIG's operating environment
as part of its continuing commitment to risk management. All
of our users' information, not just the sensitive information
mentioned above, is restricted in our offices. Only employees
who need the information to perform a specific job (for example,
our investigators and attorneys) are granted access to personally
identifiable information. Our employees must log off of the
computer network when they leave their desk. When they return,
they must re-enter their network password to re-gain access
to user information. Furthermore, ALL employees are kept up-to-date
on our security and privacy practices. Any time new policies
are added, our employees are notified and/or reminded about
the importance we place on privacy, and what they can do to
ensure that our users' information is protected. Finally,
the servers that store personally identifiable information
are in a secure environment within in a locked facility.
Correcting/Updating/Deleting/Deactivating Personal Information
If a user's personally identifiable information changes (such
as zip code, phone, E-mail or postal address), or if a user
no longer desires our service, we provide a way to correct,
update or delete/deactivate users' personally identifiable
information. This may usually be done by E-mailing our Webmaster
at oigwebmaster@mail.idpa.state.il.us or by contacting us
by telephone or postal mail at the contact information listed
below.
Notification of Changes
If we decide to change our privacy policy, we will post those
changes to this privacy statement, the homepage, and other
places we deem appropriate to ensure that our users are always
aware of what information we collect, how we use it, and under
what circumstances, if any, we disclose it. We will use information
in accordance with the privacy policy under which the information
was collected.
If, however, we are going to utilize users' personally identifiable
information, in a manner different from that stated at the
time of collection, we will notify users via E-mail. Users
will have a choice as to whether or not we use their information
in this different manner. In addition, if we make any material
changes in our privacy practices that do not affect user information
already stored in our database, we will post a prominent notice
on our Website notifying users of the change.
Children's Privacy
The OIG's Website is not structured to attract children. Accordingly,
we do not intend to collect personally identifiable information
from anyone we know to be under 13 years of age.

Neither the OIG nor any of its employees shall be held liable

and/or contained herein and assumes no responsibility for
anyone's use of the information. In no event shall the OIG
or its employees be liable for any direct, indirect, incidental,

caused and on any theory of liability, whether in contract,
