Please review the following instructions prior to completing the above documents. Do not retype, scan or alter the wording of the application in any way. To ensure you are using the most up-to-date application, always use the online copy.
Renewals must be filed annually before January 31.
Instructions for completing above documents:
The §1-109 verification form is made available for firms choosing to file under the optional Rule 722. It is the responsibility of the firm to
determine if such a verification is applicable and, that it complies with §1-109 of the Code of Civil Procedure.
The documents may be used in two ways: printed then manually filled out or filled out using a computer and then printed.
To fill out manually, download the
Application and print it out. The Application may be filled out
manually by filling in the blanks and striking out inapplicable words in
parentheses. Once completed, the Application must be signed and filed
with the Clerk of the Supreme Court along the the appropriate fee pursuant to Supreme Court Rule 721.
To
fill out using a computer, you must have Adobe Reader 4.0 or
newer. Download the Application. Once the form is displayed,
set the cursor to the Hand Tool. Use the tab key or mouse to
navigate through the form. Use an "X" or a "-"
to strike out inapplicable words in parentheses. Fill in blanks in
the form using your computer. Once completed, the Application may
then be printed and must be signed and filed with the Clerk of the Supreme Court
along with the appropriate fee pursuant to Supreme Court Rule 721.
Unless using the electronic filing option, completed forms will not be accepted electronically or
via e-mail or fax.
