Expedited Application Process – Direct Debit Industrial Radiography Certification

The Agency does not offer on-line renewal or credit card payment options. Although an application can be received via fax, authorization to work cannot be granted until the Agency is in receipt of payment. With this in mind, the Agency can offer the following:

• Complete the initial/renewal application
• Complete the authorization form for direct debit
• Fax the application, debit authorization form, and any other required documentation to (217) 785-9962.

Once the requested information is received via fax, the Agency will debit the account holders checking account for the authorized amount. Unlike a check, this transfer of funds will occur almost instantaneously, so you will need sufficient funds in the account to cover the transaction.

• After faxing, do not mail the application or anything else. It will no longer be necessary to do so.

If the fax transmission is of poor quality, or if we require additional information, we will contact you. As such, please provide a telephone number on the fax cover sheet where we can contact you.

If an immediate authorization to work is required please indicate, and provide a contact person and fax number. Otherwise, the license will be processed and mailed to the address on the application in approximately one week.
