Day Waiver applications
If granted a waiver, each licensee will be required to have
at each location a waiver which grants permission to have invoices
located at a central business location. The waiver will remain
effective unless and until a new waiver request has been approved
by the Illinois Liquor Control Commission. It is the licensee's
responsibility to have the 90
day waiver available for inspection, this includes making
sure all staff know where the document is kept at the premises.
Duplicate
License Application Form
This is different from the "Change of Business form" in that
no information changes are made to your license. You are only
receiving a replacement license due to loss, theft or destruction
of your current State Liquor License. The fee is $12 for a
license class with a standard license fee under $200 (ex.
Retail license) or $24 for a license in a class with a standard
license fee which exceeds $200 (ex. Distributor license) Loss,
theft or destruction form.
Need
a Change of Business Name Application?
If you are the owner and you are changing the name and/or address of your
business (DBA) you must fill out an Application
For Change of Business Name, submit the appropriate fee
which is $12 for a license class with a standard license fee
under $200 (ex. Retail license) or $24 for a license class
with a standard license fee over $200 (ex. Distributor license).
If you are changing your corporate name, additional documentation
is required to make the change. A copy of the amended Articles
of Incorporation, and/or proof of the Sale of the Corporation
are needed. You must also return the current State liquor
license. Note: A duplicate license fee will also be required.
One-Time
Transfer of Alcoholic Liquor Form
You can request a one-time transfer of alcoholic liquor
between the licensed premises by downloading this form.
